Substance Use Case Manager for Trilogy in Hopkinsville

The case manager will provide services to female adults that have completed a treatment program and are diagnosed with an Opioid Use Disorder (OUD) or dually diagnosed (SA and MH) in the Trilogy Recovery Oriented Housing (ROH) program. Assignments will be based on patient needs. The Case Manager will work with the client in accessing, coordinating, and monitoring necessary services to assist that individual in stabilizing in the community at his/her highest potential.
