I/DD Support Broker/Case Manager

TITLE OF POSITION: I/DD Support Broker/Case Manager

Degree Required: Minimum of a bachelor’s degree from an accredited institution in a human services field is preferred. Other related degrees considered based on experience. An Associate Degree in Human Service field may be considered based on experience.

BRIEF DESCRIPTION OF POSITION: Provide Case Management services for individuals with Intellectual and/or Developmental Disabilities. Duties include: Assist with securing and maintaining KY Medicaid Waiver services for individuals with Intellectual and/or Developmental Disabilities. Conduct annual assessments and develop person-centered plans of care (POC) to meet the waiver participant’s needs. Conduct monthly monitoring to ensure the participant’s health, safety and welfare and write clear, concise case notes. Travel is required within service region. Perform annual reassessments of POC. Review and monitor Waiver participant’s employee’s timesheets bi-weekly and troubleshoot issues. Other duties as assigned by supervisor.

LOCATION OF POSITION: Christian, Caldwell or Hopkins County. Training will take place in Christian County.
