TITLE OF POSITION: Care Coordinator
DEGREE REQUIRED: High School Diploma or Equivalent
BRIEF DESCRIPTION OF POSITION: The Care Coordinator will be a part of the Integrated Care team and work collaboratively with the Federally Qualified Health Center to help clients achieve their best health possible. The Care Coordinator will assist in motivating clients to be active participants in their health and well-being, help them overcome any barriers to accessing care, give appointment reminders, and make sure appointments are kept and follow up as needed. The Care Coordinator will also assist in making needed referrals for clients and have regular client contact to address any other concerns and client engagement in the program.
