BRIEF DESCRIPTION OF POSITION: This employee will assist in providing team based intensive case management services to adult clients who meet criteria for Assertive Community Treatment Program. Case Manager will ensure that clients receive the necessary services to maintain stability of their SMI symptoms with the goal of avoiding/reducing hospitalizations and preserving their appropriate placement in the community.
