Living is a program that provides people with disabilities the individualized
help they need to live successfully in a home of their choice. |

| Eligibility |
|
|
|
| Eligible persons
are individuals with a disability (not only persons with mental retardation),
who reside in Kentucky, a family member or guardian or a person with a disability
who lives in Kentucky. The Americans with Disabilities Act (ADA) defines
disability as someone with a physical or mental impairment that substantially
limits a major life activity which includes caring for oneself, performing
manual tasks, walking, seeing, hearing, speaking, breathing, learning and
working. |

| Who provides the
supports and services? |
|
|
| The individual or
family of the individual may choose the provider for supports or services.
This choice can either be public or private. Assistance for choosing providers
is available. |

do I need to do to receive these supports and services? |
