Supported Living is a program that provides people with disabilities the individualized help they need to live successfully in a home of their choice.
Eligibility
Eligible persons are individuals with a disability (not only persons with mental retardation), who reside in Kentucky, a family member or guardian or a person with a disability who lives in Kentucky. The Americans with Disabilities Act (ADA) defines disability as someone with a physical or mental impairment that substantially limits a major life activity which includes caring for oneself, performing manual tasks, walking, seeing, hearing, speaking, breathing, learning and working.
Who provides the supports and services?
The individual or family of the individual may choose the provider for supports or services. This choice can either be public or private. Assistance for choosing providers is available.
What do I need to do to receive these supports and services?
The first step is to complete an application. To obtain an application, contact the Supported Living Coordinator:
