| The focus of this
program is to maximize independent functioning of the individual by the
use of a staffed residence in which up to three adult clients live. The
home is staffed by Pennyroyal Center staff 24-hours a day. |

| There are specific
required characteristics of the staffed residence settings: |
|
A.
No more than three (3) individuals may reside in the residence at any given
time, and they must have a Support for Community Living (SCL) waiver.
B. Individuals served by these settings must be integrated into the community
to the fullest extent possible. They must not be isolated physically or
socially.
C. It is required that the individual's residential setting be his/her least
restrictive setting. |

|
| Eligibility for placement
will be determined based on an interview by the Admissions and Review Committee,
documentation and necessary evaluations. Immediately following the interview,
the committee will meet and make a decision regarding the eligibility and
appropirateness of the applicant in regard to the available home(s). This
decision will be communicated to the applicant/family/guardian within 72
hours of the appointment. |

| Before final approval
for residential living is made, the applicant will visit available homes
to determine if his or her needs and preferences can be met. They will be
given the opportunity to choose the more suitable if more than one residence
is available. If requested, an overnight visit can be arranged to help determine
the compatibility of the applicant. |

Our
first Staffed Residence opened in 2002 in Madisonville, Kentucky.

|
|
|
| Once a residence has been chose
by the individual, family, guardian or team, a contract will be negotiated,
setting a monthly amount to be paid. A letter will be sent to the individuals
involved to inform them of the acceptance and a date for move in will be
scheduled. |
