Case Management

One of the programs within the Community Support Program is Case Management. The main function of Case Management is to link clients who have a chronic mental illness with community resources and services and to help integrate them in community living.
The criteria for receiving Case Management are the diagnosis of a major mental illness, social and/or occupational impairments, and at least one of the following: _____1. Significant symptoms for the past 2 years or, _____2. More than one psychiatric hospitalization _______within the last two years.
